To operate a Home Services Agency, you must complete the licensing process through the Illinois Department of Public Health (IDPH). It includes:

• Complete the initial Home Services Agency (HSA) license application, including the required Bylaws, Policies, and Procedures.
• IDPH reviews the application and contacts the applicant by email if revisions      or additional information are needed.
• Once the application is approved, IDPH issues a Provisional License, valid for 240 days, and provides instructions regarding operational requirements for      the licensing survey.
• IDPH conducts an on-site survey within 30 days before the provisional license expires.
• The agency must demonstrate compliance with Illinois regulations and      successfully complete the survey to receive a full Home Services Agency      license.
